1) If a veteran entered service from the state of Kentucky you may print the attached form and fax the completed document to the Department of Military Affairs Military Records and Research Branch in Frankfort. Print Form AGO Form 26-12
2) All veterans may go through the National Personnel Records Center located in St. Louis Missouri and complete the Form SF-180. Instructions are on the following web site: http://www.archives.gov/veterans/military-service-records/
